Precision Dentistry ("we", "us", "our") operates this page so prospective patients in Columbia, MD can request a $199 New Patient Exam. This policy explains what information we collect through this page, how we use it, who we share it with, and the choices you have. It applies to this landing page only - information you give us once you become a patient of record is handled under the practice's HIPAA Notice of Privacy Practices, which we provide at your first visit.
Information you give us
The full name, email address and phone number you enter in the request form. We do not ask for and you should not submit health details, insurance numbers, payment card details, or any other sensitive information through this form.
Information collected automatically
Basic technical and campaign information about your visit: the page you landed on, the website or ad that referred you, advertising parameters contained in the link you clicked (such as utm_source, utm_medium, utm_campaign, utm_content, utm_term, fbclid and gclid), the date and time of your visit, and a non-identifying record of your device session. This is used to understand which advertising brings patients to the practice.
How we use information
To call, text or email you back about the appointment you requested; to schedule and confirm your visit; to answer your questions; to keep an internal record of enquiries; and to measure and improve the performance of our advertising. We do not use this information to make automated decisions about you.
How we share information
We share information with the service providers that operate on our behalf - our customer relationship and scheduling platform, our website and form hosting provider, and our advertising measurement providers. These providers may only use the information to deliver services to us. We may also disclose information where required by law. We do not sell your personal information, and we do not share it for cross-context behavioural advertising in exchange for money.
Cookies and advertising technology
This page uses the Meta (Facebook) pixel to measure how our ads perform, including whether a visit resulted in an appointment request. Most browsers let you block or delete cookies, and you can control ad personalisation in your Facebook and Instagram ad preferences. Blocking these technologies does not stop you from submitting the form.
Text messages
If you give us your mobile number, our front desk may text you about your appointment request. Message frequency varies, message and data rates may apply, and consent to texts is not a condition of receiving care. Reply STOP to opt out or HELP for help.
How long we keep information
We keep appointment enquiries for as long as needed to follow up and to keep reasonable business records, then delete or de-identify them. Records created once you become a patient are retained under Maryland dental record retention requirements.
Security
We use commercially reasonable administrative and technical safeguards, including encrypted transmission of the form, to protect the information you send us. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.
Children
This page is not directed at children under 13 and we do not knowingly collect their information through it. A parent or guardian should submit the form on a minor's behalf.
Your choices and rights
